Copy the above code and paste it into an Embed Code app on your page.
Replace red/all caps text within the first set of quotation marks with the URL of the content you would like to embed. (How to find URL for WCM survey)
Remove the space between < and iframe at the beginning.
You may wish to alter the height in pixels, depending on how long your survey is. For instance, a short survey may only need a height of 700px, while a longer one might need 1500.
